Part of the greater DC area, Maryland bears a number of rural areas but is increasingly a sprawling metropolitian area centered around Baltimore and Washington. This area has a rather distinct reputation; marijuana, meth, and even alcohol abuse is considered relatively low in the state compared to many other areas, however some of the worst and most chronic national problems with crack cocaine and heroin are centered around Baltimore.
This also makes Baltimore one of the most crime-ridden cities in the country. The city can actually boast several years of relative crime reduction and their homicide and robbery rates are still far higher than almost anywhere else. It is not quite clear how Baltimore and DC got to have such a bad problem with incredibly impoverished neighborhoods where drug abuse and drug dealing are done completely out in the open. But an ongoing state of morass has complicated this situation significantly, such as high-profile arrests of police officers who had become complicit in drug dealing and gang violence. A devastating consequence of this notoriety is that as many as 100,000 residents have moved away from Baltimore in the past 20 years.
As a result there is a sizable pipeline of referrals from the Baltimore Police Department to addiction treatment centers that specialize in opiate addiction treatment - almost 50% of admissions in 2008 to drug rehab facilities were referrals from the state's criminal justice system. And while there are drug rehabs in Maryland that offer treatment for a variety of addictions, these facilities have opiate addiciton treatment programs almost without exception.
